Pre-surgery preparation center
- Practice deep inhale and exhale by asking patients to sit straight on the side on the bed or lay down with the head lifted (Semi-Fowler’s position), put both hands on stomach. After that, they start to inhale slowly and deeply until the stomach is expanded, then they exhale slowly and lightly through the mouth until feeling muscles around stomach shrink. It is recommended to do this practice as regular as possible at least 5-10 times every hour
- Cough or utter aham: Ask patients to sit straight or lay down with the head lifted (Semi-Fowler’s position) and take deeply inhale through the nose and exhale through the mouth. Repeat it 3 times before inhaling as deeply as possible and holding it for a moment. After that, cough intensely and rapidly in an instant, followed by regular breathing. This exercise helps effectively coughing reaction and let out the phlegm
- Use of Incentive spirometer (Incentive spirometer)/Triflo is a help to lung exercise for reduce complications before and after surgery. This device helps patients inhale deeply and slowly which as a results strengthen muscles related to breathing and help lung function normally.
Procedures of using Incentive spirometer
1. Put patients in a straight sitting position and hold the device at chest level. Then, ask patients to inhale deeply and exhale slowly for 2-3 times
2. Slowly suck it until the ball levitates and hold in the position for 3-5 seconds (count 1 to 5) or as long as they can before letting the breath out. Repeat this practice for 10-20 times instantly and for 3-4 times during each day. Try to have patients done this at least 100 times daily as it can make lung function more efficiently.
3. In case patients are unable to sit straight, they can do this in lie-down position but should repeat it for different postures e.g. turning face up and on their side to help lung expand in every direction
Heel exercise: Ask patients to tilt heels up and down for 5-10 times repeatedly to exercise their joints that helps simulate blood circulation, reduce feet swelling and prevent obstructive thrombus
2 and 3 days after the surgery, patients can exercise their limps Lift the arms up and down, extend and gather, swirl in and out, bend and extend elbows.
- Repeat for 15-20 times as one set and for 2 sets per dayTilt the wrists up and down, make a fist and open the palm.
- Repeat for 15-20 times as one set and for 2 sets per day.Contract muscles above the knees and push the knees down as close to the bed as possible.
- Repeat for 15-20 times as one set and for 2 sets per day.Tilt the ankles up and down by counting 1 to 10 and stretch the end of feet.
- Repeat for 15-20 times as one set and for 2 sets per day.
4 to 7 days after the surgery, patients can exercise back muscles more Lay down with face and both knees up, put the arms at the sides of the body, contract the muscles around the stomach with the back close to the bed.
- Hold this position for 5 seconds and repeat it for 10 times instantlyLay down with face and both knees up, bend the hip and knees, put the arms at the sides of the body, lift the head and upper body part up, contract the muscles around stomach. Hold this position for 5 seconds and repeat it for 10 times instantly.
- Lay down with face and both knees up, put the arms around one knee and push toward the chest as close as possible and let go. Do the same for the other knee. Repeat each one for 10 times.
- However, the exercises vary by the types and procedures of the surgery as well so patients must be granted advice and approval from the surgeon, rehabilitation physician and physical therapist.
Activities to avoid after surgery
- Avoid any activity that requires complete body bending
- Avoid bending the back to pick up things and replace by bending the knees instead
- Avoid lifting heavy thingsAvoid staying on trembling surfaces
- Avoid long sitting especially on soft surface that makes the back bend but it is required to sit all day, patients must switch postures every hour.
